The following strategies have helped a number of states, municipalities, employers, and unions to expand affordable health coverage.

Changing Medicaid reimbursement rate – The Medicaid program accounts for nearly half (47.4%) of all long-term care spending in the U.S., making it a major source of revenue for long-term care employers.

Expanding public insurance coverage – Direct-care workers often have incomes so low that they qualify for publicly financed health coverage.

Employer purchasing pools – To access affordable health insurance products, some small employers and independent providers have formed purchasing pools, purchasing alliances, or health and welfare funds that give them the ability to negotiate better rates with insurance companies.

Premium subsidies – Some states subsidize small employers to reduce the cost of their health insurance premiums.

Collective bargaining – Many direct-care workers have secured affordable health insurance coverage through collective bargaining.
